Registration of Legal Instrument

 

Procedure:

1. Submit Legal Instrument applicable such as:
•    Affidavit of Acknowledgment/Admission of Paternity
•    Legitimation by subsequent marriage of child’s parents
•    Affidavit of Reappearance
•    Marriage Settlement
•    Affidavit to Use the Surname of the Father (AUSF)

 

2. For Child’s Legitimation by subsequent marriage of parents (not married at the time of child’s birth):

Requirements:

1.    Affidavit of Legitimation by subsequent marriage of parents to be signed by both parents (prepare at the LCR Office);
2.    Certified copy of Marriage certificate
3.    PSA (NSO) CENOMAR of both husband & wife
4.    Birth certificate of affected child
5.    Payment of registration Fee – P300

3. For Registration of Affidavit to Use the Surname of the father’s (Republic Act 9255) (for illegitimate child born on March 19, 2004 to present and acknowledge by father in the birth certificate)

Requirements:

1.    Affidavit to use the surname of the father (AUSF) signed by child himself (7-17 years old), or by mother (if child is 0-6 years old)
2.    Sworn attestion of mother (if child is 7-17 years old)
3.    Certificate of registration of AUSF
4.    Payment of Registration Fee – P150
